We ask that all athletes who are traveling with us download and complete this form. This is only in case of emergency while on trips, and will not be used otherwise.
It may either be completed and sent with your inquiry email, or done in session with your coach. If you have any questions or clarifications, please do not hesitate to ask. Thank you!
